ABSTRACT

Low triiodothyronine syndrome (LT3S) is an abnormal alteration of thyroid hormone levels in patients with acute and severe illnesses in certain disease states, without clinical symptoms corresponding to altered thyroid function. There is a clear correlation between LT3S and the severity of the patient's condition and prognosis. The lower the triiodothyronine (T3) level is, the more severe the patient's condition is, and combined with acute physiology and chronic health score and other indicators, it can predict the prognosis of the patient's condition. The mechanism of occurrence and development of LT3S is relatively complex. In the early stage, it may be the adaptive change of the body to the stress condition. With the aggravation and extension of the disease course, it may participate in the disease progression.Current guidelines mostly do not recommend hormone replacement therapy (HRT) for patients with LT3S. New and more unified observational indicators should be available to fully verify the effectiveness of TH therapy.

ABSTRACT

Objective@#To explore the plasma level change of soluble tumor necrosis factor related apoptosis inducing ligand (sTRAIL) in patients with systemic lupus erythematosus (SLE) and its clinical significance.@*Methods@#Quantitative real-time reverse transcription-polymerase chain reaction (RT-PCR) was used to detect the mRNA expressions of TRAIL and TRAIL receptors-1 (TRAIL-R1) and TRAIL-R2 in the peripheral blood mononuclear cells (PBMCs) derived from active SLE patients (n=26) and healthy controls. Enzyme linked immuno sorbent assay (ELISA) was used to detect the plasma level of sTRAIL in the active SLE patients (n=42), healthy controls (n=21). Pearson correlation analysis was used to analyze the correlation of sTRAIL with clinical and laboratory parameters.@*Results@#The plasma levels of sTRAIL [(82±5) pg/ml] in SLE were significantly higher than that in healthy controls [(49±3) pg/ml], the difference was statistically significant (t=4.10, P<0.01). The plasma levels of sTRAIL in SLE with inactive disease [(92±14) pg/ml], mild active disease [(80±9) pg/ml], moderate active disease [(74±12) pg/ml] and severe active disease [(83±8) pg/ml] were higher than healthy controls, the difference was statistically significant (H=18.07, P<0.01). The mRNA levels of TRAIL and TRAIL-R2 in PBMCs derived from SLE patients were significantly higher than those in healthy controls [(1.04±0.08) vs (1.80±0.25), t=2.10, P<0.05 and (1.07±0.12) vs (2.08±0.21), t=3.27, P<0.01]. In SLE with moderate and severe active disease, plasma sTRAIL levels were associated with the 24 hours urine protein.@*Conclusion@#Plasma sTRAIL may be predictors of SLE disease activity and TRAIL pathway may be a new potential target of SLE treatment.

ABSTRACT

Symptoms compatible with irritable bowel syndrome (IBS) such as abdominal pain and diarrhea may co exist in approximately one-third of patients with quiescent inflammatory bowel disease (IBD),so called IBS-like symptoms.These symptoms present as a clinical dilemma for management of IBD and diminish the patients' quality of life remarkably.IBS-like symptoms raise much concern in the research field of IBD in recent years.In this article,the latest research progress focusing on the prevalence,pathogenesis and management of IBS-like symptoms coexisting with quiescent IBD was reviewed.

ABSTRACT

Objective To investigate the prevalence of dyspepsia and the relationship between dyspepsia,psychological and social factors among the college students in Naning city,and to improve the prevention and treatment of dyspepsia in this region.Methods Rome Ⅲ diagnostic questionnaire for adult dyspepsia,Chinese college student mental health scale (CCSMHS),Chinese college student psychological stress scale (CCSPSS),Chinese college student adaptation scale (CCSAS) and Chinese college student personality scale (CCSPS) were performed through interview survey in 2 580 Nanning college students.Chi square test and rank sum test were used to compared differences between groups.Correlation analysis was performed by Pearson correlation and Logisitic regression analysis.Results A total of 2 520 qualified Rome m questionnaires were recovered.The prevalence of dyspepsia in Nanning college students was 5.36%(135/2 520).The most common appearance of dyspepsia in college students were dislike of drinking tea (86.96%(100/115) vs 79.10%(1 605/2 029)),dislike of pickled food (85.22%(98/115) vs 76.29%(1 548/2 029)) and missing meals (40.87%(47/115) vs 30.31%(615/2 029)),the differences were statistically significant (x2 =4.122,4.860,5.685;all P<0.05),while the dyspepsia was not related with drinking,smoking and taking raw,cold or spicy food (all P>0.05).The results of multifactor regression analysis showed that the prevalence of dyspepsia was not correlated with diet.Among the twelve dimensions of psychological health,the somatization,anxiety,depression,low self-esteem,social withdrawal,sexual psychology,paranoia,force,dependency,psychotic tendencies of dyspepsia group were significantly higher than those with non-dyspepsia group (x2 =16.981,21.805,12.520,13.539,6.998,6.154,15.013,9.457,10.715,4.260,all P<0.05).Among the seven dimensions of psychological stress,study pressure and development pressure were negative life events,and their of dyspepsia group were significantly higher than those with non-dyspepsia group (x2 =6.216,Fisher exact probability test,both P<0.05).There was no statistically significant difference between two groups in the seven personality,such as active,outgoing,tenacity,rigorous,altruism,affectionate and easy-going (all P>0.05).Among the seven dimensions of adaptation,the adaptation to campus life,emotion and career choice of dyspepsia group were significantly lower than those with non-dyspepsia group (x2 =8.223,8.148,5.713,all P<0.05).While there was no statistically significant difference between two groups in the other four dimensions of adaption,such as relationships,learning,ego and satisfaction (all P>0.05).The results of Logistic regression analysis in psychological health,stress and adaption of dyspepsia group and non-dyspepsia group indicated that dyspepsia was associated with somatization (odds ration (OR) =1.610,95% confidence interval (CI) 1.012-2.559),anxiety (OR=1.955,95%CI 1.216-3.142) and the study pressure (OR=2.159,95%CI 1.106-4.213).The results of Pearson correlation analysis in study pressure,somatization and anxiety of dyspepsia group showed that both somatization and anxiety were correlated with study stress in dyspepsia group (r=0.314,0.323;both P<0.05).Conclusions Dyspepsia is a common symptom in Nanning college students.Students with dyspepsia have different degrees of psychological problems which are mostly anxiety and somatization.And study stress as a negative event is the major stress factor.

ABSTRACT

Objective:To establish an optimum preparation process for Hongteng decoction hollow suppositories. Methods:An orthogonal design was performed to screen the proportion of drug and base(A),the temperature of mold filling with drug and base(B)and stripping time(C),and the appearance and melting time were used as the indices ,the best preparation technology of the hollow suppositories was optimized. Results:The optimum preparation technology of the hollow suppositories was as follows:the proportion of drug and base was 1 ∶2,the filling temperature was 40℃,and the stripping time was 30 min. Conclusion:The optimum preparation technology of Hongteng decoction hollow suppositories is simple and feasible.

ABSTRACT

Objective To review the clinical features of chronic daily headache (CDH).Methods The clinical data of 128 patients with chronic daily headache,including general condition,characteristics of headache,concomitant symptom and disability were analyzed retrospectively.The features of primary chronic daily headache (PCDH) and medication over-dose headache (MOH) were compared.Results Among 128 cases females accounted for 79.7% with an average age of 45.2 years and 88 patients were associated with drug overdose.The symptoms included nausea (68/128),photophobia (75/128),phonophobia (102/ 128),depression (77/128) and irritability (93/128),sleep disorders (94/128),dizziness (75/128),emotional irritability(58/128) and depression(21/128).The migraine disability assessment questionnaire and headache impact test-6 scores showed that disability was resulted from the severe degree of headache in 62.2% (51/82) and 73.2% (82/112) of CDH patients respectively.Compared with PCDH patients,the MOH patients had older age (t =2.59,P =0.011),longer duration (t =2.48,P =0.015) and severer degree of headache(t =5.58,P =0.018),and chronic migraine (t =11.95.P =0.001) was the most common primary headache type.Conclusions Most CDH patients are middle-aged women,with moderate to severe pain,usually complicated with depression,dysphoria and asomnia.Chronic daily headache patients are commonly associated with drug overdose.

ABSTRACT

BACKGROUND: For decades, liposome drug carrier has been used to enhance drug stability and efficacy, reduce drug toxicity and adverse effects. However, they fail to provide long-term delivery due to insufficient stability. Studies have demonstrated that silica is not toxic, with chemically inert and biological compatibility, and can be used as modified material. OBJECTIVE: To characterize the silica coated liposome and investigate the controlled release property. DESIGN, TIME AND SETTING: In vitro observation. The study was performed at the Nanomedicine and Biosensor Laboratory, Biomedical Engineering Center, Harbin Institute of Technology from May 2007 to June 2008. MATERIALS: Dipalmitoylphosphatidylcholine (DPPC) was purchased from Nanjing Kangsente Chemical Engineering Company; tetraethylorthosilicate (TEOS) was purchased from Aldrich, USA. Doxorubicin (DOX) was purchased from Beijing Huafeng United Technology Company; Sephadex G-50 was purchased from Amersham Biosciences, Sweden. All other chemical agents were of analytical purity. METHODS: Liposome was formed from DPPC following the precipitation of silica by sol-gel method. MAIN OUTCOME MEASURES: Zeta-potential and dynamic light scanning were used for zeta-potential measurement and particle size distribution; transmission electron microscopy was used to collect the image of particle morphology; Fourier transform infrared spectroscopy (FTIR) was used to display chemical characteristics of Si-O-Si structure; Spectrophotofluorimetry was used to determine DOX regression equation and was further used for calculation in drug encapsulation efficiency and in vitro release. RESULTS: ①Silica coated liposome was successfully prepared. ②FTIR proofed the presence of Si-O-Si at 1 166, 1 080, 859 and 526 cm-1. ③The DOX encapsulated silica coated liposome had encapsulation efficiency of 72.4%. ④Drug release profiles showed that sustained release of DOX was achieved after modification of silica on liposome. CONCLUSION: With Si-O-Si as protective layer, the liposome has increased stability and prolonged drug release.

ABSTRACT

Aim To prepare for psoriasis transgenic animals, psoriasis susceptible gene was constructed. Methods Extract gDNA from blood preparations of psoriasis patients, screen positive gDNA owns HLA-Cw*0602 gene, then copy the whole sequence and link it to pMD-19T Simple Vector. Results The identification showed the construction of psoriasis susceptible gene was sucessful. Conclusion The successful construction of HLA-Cw*0602 gene will lay the foundation for development of transgenic animal models of psoriasis.
